Fees
My fee is $125 for a 45-60 minute appointment.
Cancellation Fee: If you cancel with less that 24 hours notice or do not show up for your appointment, you will be charged $60. This fee is not covered by insurance.
Sliding Scale: If you are not using insurance and are not able to pay my full fee, you are welcome to discuss a sliding scale fee with me.
Insurance
I am an in-network provider for Blue Cross Blue Shield. Vermont Medicaid (Green Mountain Care), and MVP. As a contracted provider I have agreed to accept their contracted rates for my services.
For all other insurances, I ask that you pay my full rate at the time of service. I can provide a “paid invoice” that you can submit to your insurance company for possible reimbursement. It is your responsibility to determine whether your insurance company covers out-of-network providers, and at what rate.
Co-pays, co-insurance, and deductibles: I ask that you pay these fees at the time of service.
Payment
I accept credit/debit cards, cash, or check as forms of payment.
